DETAILS:

On Tuesday 20 February (2 pm – 3 pm), The Pennine Acute Hospitals NHS Trust will be hosting a free ‘talk and tour’ of the new Crumpsall Vale, Intermediate Care Unit at North Manchester General Hospital.  The event will be an opportunity for staff, public and our Trust members to speak with staff and patients and to learn more about the unit.

Crumpsall Vale intermediate care unit is a 24 bedded unit and is located on two floors, separated into three pods of eight beds, eight residential beds (green bay), on the ground floor and sixteen nursing beds, (yellow and orange bays) on the lower ground floor. Therapy intervention is offered seven days a week with the aim to improve patient and carer experiences by providing better access to the right intervention, at the right time, delivered by the right health or social care worker.

The service is provided by a multi-disciplinary team of Nurses, Allied Health Professionals, Social Worker and General Practitioners from The Pennine Acute Hospitals NHS Trust and Manchester City Council.

The Intermediate Care service offers holistic and comprehensive assessments of people so that increasing numbers will be able to maximise and maintain their independence in the local community. The aim of the service is to deliver all services through professional groups within teams so that they truly place people at the centre of this care pathway.

Intermediate Care supports the transition between illness and recovery and in North Manchester; Intermediate Care is primarily, although not exclusively used by older people and especially those in advanced older age.

The aim of the social model of rehabilitation for Pennine Acute Hospitals NHS Trust is to focus on services that are person centred, promote independence and facilitate appropriate safe discharge arrangements.

The talk and tour on Tuesday 20 February 2018, is part of the Trust’s series of ‘Medicine for Members’ events and will be held at Crumpsall Vale, (the unit is a brand new building, located just left of the Occupational Health Building and we will meet in the reception area),  North Manchester General Hospital, Delaunay’s Road, Manchester, M8 5RB.
